Complete this onboarding form to get your clinic set up on the AirwayZ patient education platform.

Branding

Upload your logo and choose your brand colors. Your colors will be applied throughout the AirwayZ portal for your clinic.

How to sample colors from your logo:

Brand Colors

Choose two colors that represent your clinic. Both colors must be dark enough that white text is readable on top of them. Test your colors at webaim.org/resources/contrastchecker — you need a minimum ratio of 4.5:1 with white text.

Primary Color *

Used for headers and backgrounds. Should be your darkest brand color — navy, deep teal, charcoal, etc.

Secondary Color *

Used for buttons and accents. Should be a medium tone, noticeably different from your primary color.

Set a 4-digit PIN for your staff to access the daily patient dashboard. All staff at your clinic share one PIN.

What Happens Next

Within 24 hours — You will receive an email from the AirwayZ team with an update on your onboarding progress and any follow-up questions.

DNS setup — We will contact your DNS contact to coordinate your clinic subdomain setup.

Data agreement — Your legal signatory will receive the data processing agreement for signature.

Portal access — Once setup is complete, all listed staff members will receive their login credentials.
